The adaptation in sexual relations and its balance are one of the most important components of marital happiness and satisfaction. The purpose of this study was to develop a structural model of sexual performance based on gender identity and sexual self-concept in women using body mass index (BMI). Methods: In this cross-sectional descriptive-analytical study, the serum procalcitonin levels were measured in 120 patients with suspected appendicitis who underwent appendectomy surgery. The histopathology reports of appendectomy was the basis for diagnosis and the serum levels of procalcitonin were measured by chemilumine scence method. Data were analyzed using SPSS software (version 19.0, SPSS, Chicago, IL, USA). Results: Based on the results of fit indices, the model has a good fit (χ2 / df = 2.81, CFI = 0.92, RMSEA = 0.06). The results showed that the path of sexual identity, sexual self-concept and body mass index to sexual function is negative and significant (P <0.01). Also, the path of sexual identity to the body mass index is negative and significant and the path of sexual self-concept to the body mass index is positive and significant (P <0.01). In addition, the results indicated that body mass index did not mediate the relationship between sexual identity and sexual self-concept with sexual function in women (P <0.01). The findings also showed that sexual identity, sexual self-concept and body mass index explained 71.5% of the variance for the sexual performance. Conclusion: According to the findings of this study, the persons' perception of their sexual or emotional desire as gender identity and beliefs as a person's sexual self-concept have a predictive role in the women's sexual function. It is suggested that in counseling and treatment of sexual issues, body mass index should be considered as an important factor in increasing female sexual performance. M3 ER -
